The Iowa Clinic takes very seriously the role of safeguarding personal health information. On August 6, 2020, we discovered that a clerical error involving our electronic address file caused a form letter to be sent in envelopes with the wrong recipient names. The form letter contained information about a change within our Pulmonary and Critical Care Departments, and there was no individually identifiable information within the form letter. As no financial information or identifiable information other than name was involved, we do not think individuals need to take additional steps to protect their information. We are notifying the affected individuals in accordance with our HIPAA policies. Appropriate steps are being taken to review policies and procedures and to ensure this does not happen again.
