Work with you to develop the best treatment plan or care plan possible for you

Be respectful of your privacy always

Share information in a way that you understand, especially when it comes to benefits and risks associated with treatments or medications, to allow you to make informed decisions regarding your health

Respect the role of my care team in my health and healing and treat them with kindness and consideration

See members of my care team as people and refrain from making derogatory or rude comments to any member of my care team for any reason

Be respectful of my care team and their time by keeping scheduled appointments and avoiding unnecessary last-minute cancellations

Always tell the truth about your current health and previous health history

Take an active role in your health

Follow through with care and treatment plans that are agreed upon with your healthcare provider and notify us if there are any concerns or questions

Keep us informed of any changes to your medications

Transfer medical records from other healthcare providers when necessary

Stay informed about my insurance coverage

Tell you if I have trouble understanding or reading the information provided to me

Let you know if I am uncomfortable in any situation due to past trauma

Tell you if I have family members, friends or other advocates involved in my healthcare
