You need to be seen in clinic regularly
We want to see you in clinic every 6 – 12 months and more often if you are not doing well. These office visits are important to keep you healthy and to make sure your treatment plan is working for you.

Let us know if you are feeling sick
Your health is important to us. We will arrange for an urgent visit to help you feel better.

The Patient Portal is an easy way to communicate directly with your IBD team
You can view test results and send and receive messages concerning your health and treatment plan.

Keep us updated with any changes in your health or hospitalizations
Please bring records of any discharge summaries, imaging studies or blood work to your appointments.
